Transition from children’s services to the world of adult services is often a complex and daunting time for young people and their families.
Getting it right at the early stages will help the young person to settle into a seamless phase of their life which for the best of us, is fraught with anxiety as we move from being dependent on our families to making key decisions for ourselves.
